Username Recovery

During the re-credentialing process, a provider may not remember the username they used to complete the application. Using the "Forgot your username?" tool when completing the re-credentialing process makes recovering your username simple and easy.

NOTE: The Username Recovery tool is only available for providers submitting a re-credentialing application to certain payers. If you don't see the "Forgot your username?" link, you may not be able to recover your username with this tool. Please contact our support team for assistance in recovering your username.
  1. Once you arrive to the login page of the payer re-credentialing process, click the "Forgot your username?" link to open the recovery tool
  2. Enter your individual (Type 1) NPI number and either the date of birth or last name for the provider. If you don't find a match using the NPI and date of birth, try to use the NPI and last name only.
  3. If a match was found, the email address(es) registered to that provider will be displayed. If you have access to those emails, click the SEND button. Within a few minutes, you will receive an email with the username associated with that email address.
  4. If a match is not found or if you no longer have access to the email address(es) listed, you will need to contact the DentalXChange support team for help recovering your username.
